I’m unable to send comments to an approver along with my request.

I’m using the simple approval path to send a document for approval that I have used a template to create. That person is added as the approver on all instances of the template. When they send back their comments, I can see those. However, when I reply with a revised proof I cannot add any type of comment. The only thing I can do is send the revised document. Is there a way to comment in the approval path directly without commenting on the document overall? And, is there any way to share that same document for approval (in an approval path) without making that person an approver on every instance of the template use?
