Plan advice

Hi,
I have a tuition centre and what I'd like to be able to do is to have new customers complete an enrolment form / agreement contract in the centre ( or email a link to those who sign up at a later date). The form would need to have fields that are compulsory to complete so that I always capture all the information that I need. Can you advise which package would cover my needs please?
