Position of an image moves after saving (anchored frame)

I have an issue with my structured document. Whenever I try to move an image to its correct position, after saving, the position changes. Let me illustrate.

The house icon is now in the place where I want it to stay. However, after I save the document, this happens (next image).

Position of the image moves? I've tried anchored frame settings (Anchoring position > at insertion point) and seemingly everything I know. Our company manual has a lot of these inside paragraph icons, so help would be much appreciated.
