8.5 x 11 book (novel) template

I am trying to set up the internal InDesign layout for a book I've written. I have helped a teacher friend of mine put her's together using the free ID "poetry-novel" template. That worked well.

But, for my material, I need help figuring out the internal specs for a book that, when finished, would trim out at 11.5" x 8.5 with a .5" spine (perfect bound). I have come close on my own efforts, but really need a template that would give me margins, bleeds, and page gutters .. et al.

With that, I can pull in images and graphics and create style sheets and such that would facilitate text flow.

Let me know what you think, and any applicable charges that you might require.
