To elaborate on Willi's answer, when the spot white shape is set to overprint, the pdf will retain the spot color under the image or object. Alternatively, you can place the spot color elements on a bottom InDesign layer and export to a layered pdf (overprint unnecessary here). If you have white elements above the color, create a white swatch (not a spot color) to KO the color and allow the spot white to show. Check the layers in Acrobat. Note, the Acrobat Output Preview tool will correctly show the spot color when you go to- Show: "Spot color" (but won't when you use show: "All"), or you can place the pdf into InDesign and go to Object> Layer options to confirm the spot white is behind the image. I like to tint the spot white blue, so it's easier to see on-screen. The press operator should be able to use the spot layer and print it in the desired order, depending if the label is intended to be viewed from the inside or outside of a window.
