Buttons and Forms (LOCKED: Duplicate Post)

Hello

I am working on making a multi-page form interactive.
CS InDesign 2023 v18.1
I have spent hours trying to figure this out by trial-and-error. Deadline is looming so I am reaching out for help.
(screenshot below) I have set up these as checkboxes, with different names. I need them to be checkable--but also uncheckable as we want people to be able to correct something they might have checked in error.

Also, there are 4 boxes here. Need to set it up so that people can choose only 1, 2, 3 or all four of the statements.
I appreciate any guidance from the community.
