Create Form, Export to PDF, then export PDF to CSV

I have a unique situation where I need to create a Form in InDesign and export it to PDF—and once the form is filled out in the PDF version, I'd like to be able to export each field title and field to a CSV export correctly. I have exported the PDF to Excel, but the format is all sorts of weird. Is there a way for me to specify, in Indesign, which information is a "header" and which is "data"? I would like the Field Titles to populate in the header and the answers for each to populate in the subsequent rows below the headers. If anyone has any other suggestions I'm open. I appreciate any and all help that I can get.
