Endnotes - Superscript and Title

I have a document that I pasted from word to Indesign CC that has endnotes. The in text citation is superscripted but I want to automatically format the actual endnotes at the end of the document. I need two things:

1. Set all the numbers at the beginning of each endnote to superscript. I have tried using the instructions from this post but it doesn't seem to do anything.

2. Add the title "Endnote" to the top of the endnotes section. I can do this by adding a text box above the endnotes area (it is in a different text box than the actual article) but I want to add it to the top of each endnotes section automatically when I add endnotes (if possible).

If you want to retain the internal link between the reference (the superscript number in body text) and the footnote/endnote, then do not copy/paste the text!

Instead, use the Import Options in the Place dialogue to import the Word document. This retains the connection between the reference and the footnote/endnote and you can then intelligently manage and format everything. Use the various utilities and controls under the Type menu to control the placement and appearance of them. Use a paragraph style to format the footnote/endnote, and a character style to format the reference.
