Forms in InDesign

Is it possible to create a list field that has static fields and a field where you can enter an amount all for the same list menu. For instance . . . Not Used, Included, By Others and a field to enter a dollar amount all in the same box.

    Hi Ramona:

    InDesign has a limited number of features you can use to create forms that are then converted to PDF. You can create a combo box in InDesign but it doesn't have the checkbox to allow typing by the person filling out the form:

    You can add it to the box when you get to Acrobat:

    In InDesign:

    In Acrobat, after activating the checkbox to allow user to enter custom text.

    That often means finishing the math-ability of the form fields in Adobe Acrobat.
