How can I use Photomerge to create a document with a different photo on each page?

I want to create a photo book:


What I already have:

  1. 
A Blurb book template for InDesign

  2. A .csv file with all the photos
  3. A frame on each A-parent frame 

How  can I ensure that there is a single, different photo on each page

- with captions if possible?

    Right, so:

    1. Create a single-page document in InDesign
    2. Draw the desired frames = e.g. photo and image description
    3. Create the merged document
    4. Go to “Document Setup...”
    5. Tick “Double Page”
