How do I add a text form field to a question with radial selection?

I have a few questions on my survey that has a fillable "Other" option.

Example:

Where did you find this posting?

Word of Mouth

Facebook

Other:

 

I want 2 things to happen:

  1. When "Other" is filled in no other options can be selected
  2. I want the "Other" copy to disappear when someone changes their choice after they fill in "Other".

If I understand you correctly, you can create the radio buttons for Word of Mouth, Facebook, and Other. Then create a text field for OtherText and make it Hidden. 

Action for Other: Show OtherText

Action for both Word of Mouth and Facebook: Hide OtherText

What you want to happen is somewhat contradictory. You're saying if other is filled in, no other options can be selected and also saying you want other to disappear if someone makes another choice. David is providing good guidance but it's not clear what you really want to happen.
