Hyperlinking data merge values

Hi All, 

I need some help please.

I have the below data merge values, what i want to do is take the value from the first line, (the month) then take the day value underneath it and hyperlink it to a page called month-day.

Does anyone know how this can be done please?

Do I have to label all the fields as that will take me ages.
