InDesign data merge, missing data fields

I'm trying to do a data merge, I've selected my Data Source (a .csv file), but not all of the data fields (column headers) are showing up in the data merge box. Any ideas what I could be doing wrong?

    Likely one of the missing fields have returns in them, or another disallowed character. If you open your merge file in a text editor, is it all there?

    A screen shot of your data merge box, and some information about what is missing would be helpful in helping you.

    Sometimes csv files from some regional versions of excel use semicolons instead of commas. If all of your headers seem to be together in the first field name, try saving your file as tab-delimited text instead of csv.

    I had the same problem with the tab-delimited text file. Here are all of the data fields that show up, but I have a total of 34 data fields.
