Indesign data merge problems

I have a very simple merge. But the 'Preview' and 'create merged document' are greyed out. I am not sure of the reason for this.  Can anyone help?

Based on your screen shot, it looks like the field names in the Data Merge Panel have not yet been placed on the page.

If they had, you would see a page number in the panel next to the field name. 

You can see in the screen shot below, only three fields have been added, (they each show a number 1 in the merge panel) so only those three will be merged.
