Index indicating chapters names per reference

I would like to know how, in addition to the pages of each item of the index, to indicate the full name of the "chapter" in the references. Each chapter is in a different document from the book.

I found how to reference the chapter number in the index through the sections, but since I have many different chapters, and the topics can be better illustrated if besides the page number there is the chapter name in the index, I wanted to know how I could do that.
