index not picking up entries

I have a book with some dozen files. The files are all open, synchronised, page numbers updates and fully preflighted. I Have

Generate Index.,Include Book Documents.Enable Replace Existing Index.

Yet, the index fails to pick up numerous entries.

See image, there are many many entries for ‘Kevin’ but only one picked up.

After laying out a book and generating an index from that layout, my new managing editor decided she wanted to add hyphenation to the entire book (something I only do selectively for space reasons), so I decided to save the original layout as a separate document and renamed it, adjusted the layout due to the addition of the hyphenation and generated a completely new index. The new index included some sub-entries, but in some cases not all sub-entries, or no sub-entries at all. None of the index tabs were deleted, but the resulting index didn’t pick-up all the sub-entries. What would cause the discrepancy between the two indexes when the text was unaltered except adding hyphenation?

I think I used Add All…. I have deleted ‘Immediate Family’ and recreated it, Added ‘Kevin’ as a reference (no option to ‘Add All’). Now it finds no entries when I regenerate the index

I am now totally confused.
