index not picking up entries

I have a book with some dozen files. The files are all open, synchronised, page numbers updates and fully preflighted. I Have 

Generate Index.,Include Book Documents.Enable Replace Existing Index.

Yet, the index fails to pick up numerous entries.

See image, there are many many entries for ‘Kevin’ but only one picked up.
