Index panel reference entries missing after document conversion?

I have returned to work on a book after several years away from it. For some reason Reference entries in the index panel no longer appear. The index updates correctly-so they are somewhere, but I do not have access to the actual reference items in the index panel. Is this after the recent file conversion? I really am not anxious to reconstruct the entire index and am hoping someone can set me straight.
