Special TOC in ebooks

The book I'm working on has boxed titles for each chapter. To create these, I treated them as an image so that I could create the lined box. But when I create the ebook TOC those titles are not linked to those pages. How can I fix this? Or is there a work around?

Also, the author wants the titles in caps and small caps, but wants the ebook electronic TOC in regular type. Is there a setting that can do this without having to know programming?
