why is my digitally fillable exporting blank

Hi all,

I'm having trouble when exporting my digitally fillable form from indesign, I am saving it as interactive the boxes just don't pop up. Does anyone know how to fix it? I've attached a pic of it.

It appears that you have one huge form field overlaying the rest of the form but as Eugene points out, we can't really tell without the files.

I'm not sure anyone can help based on the supplied description and image just showing a blank.

You can attach PDFs and IDML files to the forums.
Can you please supply example files and/or steps to reproduce?
