Lightroom duplicates some photos

Hi! I don't know what happened with one of my imports but now LR is displaying some photos twice, although they are from the same source file. It's very annoying especially since I tried to delete one copy, and it deletes both...

Anyone sees what might have gone wrong?

Please show us a screen shot.

They could be Virtual Copies, a VC has a turned up corner at the bottom left hand of the thumbnail.

Or exported to a different file extension, JPG or TIFF, and you have Added them to your catalog.

LR does not arbitrarily duplicate images. It take user intervention to do that.

They are not virtual copies, and they have the exact same file name with the same extension. Even when I click on reveal in finder, both point to the same source file. The only difference I noticed is that the first one has in the Quick Develop tab a saved preset of "Default Settings" and the second version a saved preset of "Custom".

Do you use any Import Preset to Copy the files?

Did you create the catalog from scratch (or) Merged two different catalogs?

One of the reasons could be file getting copied and it got imported again.

Also, is it possible for you to share the catalog for investigation?
