LR not updating images

Using LR (13.5.2) and PS (25.12.0) WIN11. When "save a copy" back to LR the images do not update. Yet, if I look in Explorer, or open that image again in PS, updates are there. I have reset LR several times, and it does not help. This will happen to any image I try it on, so it is not just one image. Ideas? Thanks.

This is as expected. As the name suggests, "Save a copy" saves a copy of the Photoshop image, the original remains unsaved. Lightroom Classic keeps a link to the image it has sent to Photoshop, so it can add that image to the catalog when it is saved in Photoshop. Lightroom Classic does not know anything about any copies you saved, so these are not automatically added to the catalog. So either use 'Save', or import that copy yourself.

I can save, save as, save copy...LR does NOT have them even when I try sync folder, zero. As I stated, Exlporer will list it, and I can search or Open Recent and get the image, really seems to me to be an issue.

It's not an issue for almost everyone, so this must be something on your system. What is the file you send from Lightroom to Photoshop? A raw file or some other file format? And where in Lightroom Classic are you when you do this? A folder? A collection? A smart collection? Problems like this are often the result of being in smart collection that filters on raw files (the returned file is not raw, so it won't show in that collection), or having an active filter that does the same thing.
