Multiple files in created in catelogue

I open Lightroom today and discoverd that I have multiple files of the same image (DNG). Most of them are in staks fo 11. This across multiple folders of images. A folder of 50 or so images now contain over 3000 images. Has any one seen this before? Thoughts on why or how this happen.
