Photos not stacking after editing in PS and saving back to LR

When I take a photo from Lightroom "edit in PS" do some changes then save it back to lightroom the photo does not "stack" with the original in the folder it came out of. It kind of creates another folder with the same name. when I ask it to "show in parent folder" it says it is there. Same for "show in explorer" it says it is there but still shows on the left side in a separate folder with the same name. Things I have done so far to try and correct the problem. Uninstalled both LR & PS and re Installed. "reset Preferences" in both programs to original. Researched online to try and figure out exactly what the issue is. Is it a PS issue? or LR ?? not sure. I have latest versions of both on latest Windows software
