Duplicate name on thumbnails

When hovering over thumbnails, I see a dulpicated file name. i.e. Visit to London Eye jpg. (twice below each other). How to I revert to just the one name please? I also have the file dimensions (3872x2592 etc), which is fine. Can anyone advise please? David
