Images revert to originals in LR

working in LR every 10 or so images I work on ill go to finish them and crop them and it is reverting them back to the original image for some reason. This has been hapening since the update consistently,  also ill apply a mask to an image work on it for a while then all the sudden the mask will delete itself and i have to scan and apply the mask edits again. Very frustrating and disruptive to workflow.
