LR Classic Missing Galleries after Upgrade to v9.4 on MacOS High Sierra 10.13

I followed the instructions for catalog-upgrade and discovered that there are dozens of missing galleries. I repeated the catalog-upgrade steps with multiple backups. Same result.

How do I fix this?
