How to make a 3D object with holes

Hi,

I saw this (link below) on a stock photo forum and would love to learn how it was made.

Although PS can do some interesting 3D things. Like beveling, extruding, applying surface properties, and lighting. That sort of distortion on the interior part of the object would need to be done in a program like Cinema 4D or other 3D programs similar to it.
