Image disappearing after brush stroke

When I make a brush stroke the image disappears and is replaced with this chunky transparency looking grid. What's going on? New Win11 high spec PC and intall of PS (25.3.1).

Please take a screenshot of the entire window on your screen and make sure that the Layers panel is expanded and clearly visible in the image. It is very important for us to see the Options bar, which is located right below the menus. Additionally, please ensure that the Brush tool is active when you take this screenshot.
