Why is the "replace color tool" not available for use in every photo?

Things I'd check:
* Is the image Indexed Color (Image > Mode > Indexed Color...)? That would limit color replacement.
* With the image that doesn't have the tool available, have you selected the right layer? If you have an empty layer or a text layer selected, the replace color won't be available.

Hey, Dean -- I had to change my 16-bit NEF to 8-bit before I could change Mode to Indexed Color.

After that, Image>Adjustments>Replace Color... was available.

Once the original raw file has been rendered into Photoshop, it's no longer a NEF (or DNG, CR2, ORF etc). The raw data has been processed and rendered into Photoshop, the raw data only exists in Adobe Camera Raw.

@DeanUtian suggested that if the image was in indexed colour mode, it should be changed to RGB, which would be 8 bpc. He wasn't suggesting you throw away most of the colour data by converting to indexed colour mode.
