Running Action - Could Not Process The Following Files

I run actions on a regular basis, for example to generate proofs to send to clients - say 400 images. The action works fine and processes all the images. However on completion I get an error message every time stating, "Sorry I could not process the following files". Any idea why this is being shown when it's worked perfectly? Since updaitng my Mac OS I cannot scroll down to click "ok" to close the dialog box, so have to quit Photoshop altogether and re-open it whenever I batch processed images.

Which files are listed in the alert? 

Could you plese post a screenshot of the fully expanded Action in the Actions Panel? 

And one of one the erroneously alerted files with the Channels and Layers Panels visible?

Thanks, will upload later. All I'm doing on the Action is making the image smaller (lowering to 120 DPI resolution and exporting as a JPG) then closing the image without saving. The error message lists every single image that Action is used for - so in my most recent case it was 400 images.

Run it manually on one image and see what the problem is. If you use Batch, you can write errors to a log file.
