You need to embed the CMYK profile and view it in an application that supports color management.

In addition, a lot of consumer-oriented applications don't support CMYK at all (even if color managed). Those applications may display CMYK files with random colors, or not at all.

Where do I find the option to embed it?

I opened it with the "preview" macOS app

I don't do Mac so I may be wrong, but a little googling suggests that Preview is one of those apps that don't have reliable CMYK support. Like any consumer-oriented image viewer everywhere, it's built for and intended for RGB files. CMYK is strictly for commercial offset printing, not something the average user will ever come across or ever need.

Photoshop is the proper tool for any kind of CMYK work. It will always handle CMYK data correctly.

As long as you haven't made any changes in Photoshop's color settings, the profile will be embedded. If you have made any changes, please show a screenshot of color settings so we can see what those changes are.
