A pretty good scam?

In a fb page about photoshop elements a user posted the image below.  What's interesting is that I completed a simliar survey, which was obviously from Adobe.

A big clue that this is a scam is the part about "created by yahoo mail"-  I highly doubt Adobe would use that. 

SO WADDA THINK, A SCAM?
