PSE 2022 Smart Brush Usage

The Smart Brush is a very confusing tool for me. Whenever I use it I keep expecting to see the "accept / reject" icons   after I make the selection, but it's not there!

I click on the effect preset I want and then make the selection I then have to double-click on the effect in the tool panel to accept it and create the effect. Am I just using it wrong? What say you?
