Audio 1 not working in Premiere Pro

Audio 1 track appears different from other audio tracks.  There is no padlock icon, and the toggle, mute and S items are missing.  Therefore, any new clips I put into the timeline have no sound.  Same for the Source screen; it plays video but not audio.

Somehow, it is OK now.  Audio was not playing in Source panel and when I put a new clip into timeline.  Everything else in timeline was ok for audio.  So, no screenshot.  But, there was no padlock icon to the left of A1 and to the right of A1 were no toggle, M and S icons.  Thanks very much for helping me.  My version is 2015.2 by the way.

I have the same problem with the new update Premiere Pro CC 2017.0.1

It was working fine with 2017.0.0.  But after the update, I cannot import m4a audio files.  And existing projects with these already imported to the timeline show it in red with 2 double green lines through it.
