Audio & video unsynced after closing project

I had a shoot with 2 camera's, both had a tentacle sync-unit during the shoot.

All the video and audio is synced in the Tentacle sync sofware and imported to Premiere Pro.

 

After my editing is done, I close my project. 

The next morning, the only thing I see is this:

There are no reports of missing files etc, but some of the clips look like these.

They don't play any video, the screen is just black.

There are also audiofiles with these.

Some other videoblocks and audioblocks still contain content, but these are not the timecodes I used. (Shifted for random times,..)

And some other don't have problems at all.

I can't find a logical way to understand this issue in the software.

 

This is the 3rd time this happens to me (with different versions of Premiere), and I don't have a clue what this could be.

Can anyone help me out to fix this?

Some extra information:
The extra weird part is there is no logical way in when this happens and when not.
Some parts took other timecodes in het 'video' blocks that are in the edit. So sometimes it's hard to find where the mistakes are.

Also, There is a separate timline (from the import XML from Tentacle Sync.). This timeline keeps synced with working videofiles.
Audio and video copied from here on, Is off-sync/drifted in/out-points,...

 

So, Premiere knows what the footage is in the original Synced Timeline...
