cannot see video on my premiere pro project

i imported a mp4 video clip into Premiere. When i try to move it to the timeline, it only puts the entire video on my audio track. Cannot see the picture (video)

Funny thing is that i can see the picture (video) along with audio in the source window.

But on the timeline, it only allows me to use  the audio track.

Source video window also has "video only" option. Did try that also. No luck.

Could it be a codec problem?
