Ctrl-D didn't add my default transition

I'm between clips on the timeline and hit Ctrl-D, expecting the default transition to appear, but nothing happens.

So I go to the Effects Panel and drag my default transition between the clips and the transition appears.

Why didn't the shortcut add the Transition?

   I get the transition if no clips are selected. But that puts a transition between all my clips, doesn't it?

If I select the clip and put the playhead between the clips then hover over the end, til the icon appears; the the double arrows, then click Ctrl-D nothin happens.

No.

Just select the cut with your mouse, not the clip(s).

Do mean is one selected?

I just opened Pr, went to a Sequence, selected the second track in the sequence, and clicked Ctrl-D and nothing happened. The Play head was not between the tracks but I don't think that matters, but just in case I put it between the clips and still nothing happened.  There are Effects on the track, as indicated by the little Fx icon in the top left corner of both V and A tracks.

I checked my Effects and Cross Dissolve is set as Default and I am able to drag it between the clips on the timeline.
