Exported video is not matching sequence settings

I can't get Premiere to export a 1:1 video regardless of my sequence settings being 1:1.

This also happens with other video sizes like 16:9, etc.

What am I missing here that is causing these issues?

You are using the wrong pixel aspect ratio.

You have it set to 0.9091 which is DV ntsc.

It should be set to square pixels 1.0.
