How can I export labeled name and discrete audio channels?

 

I want to export ProRes MOV which have labeled by custom name(for example : LFE screen) and discrete audio channels one channel per one track, like above screen shot.

I have a lot of tries like... bus, audio types change and render 'all time line tracks', render 'discrete all channels' in Davinci or Premiere, but I can't still figure out, how can I export exact same appearance like above screen shot. I need to put custom name into each discrete channel.

Could you anyone help me?
