I had this issue too but it was due to my 2nd monitor being connected whislt trying to use the audio channels window. I unplugged the HDMI from my Macbook Pro to have Premiere open on my macbook pro screen instead of monitor screen, reopened the audio channels window and the ok button had reappeared.

I can confirm as with others in this thread, that hitting the Enter key after adjusting your preferences does work. Grateful! But it is a super annoying bug and surprising that Adobe hasn't ironed it out yet since the original post was in 2018...

Wow, yes - this bug was making me CRAZY!!!  Still a problem even when I upgraded to very latest (15.1) Thank you ERIC for suggesting hitting enter. That does work.  I kept having to force close premiere every time I tried to open the audio channels.

If nothing helps, just pressing the enter key after fixing your audio channel issues is what worked for me.

This is the first that I’ve heard of this. Maybe try re-setting preferences on have the app closed. On Windows hold down shift/alt/control and click on the app icon to open Premiere. Keep holding the keys and let Premiere delete preferences. Damp procedure for Mac but hold down Command/Option/Shift when starting. Then re-open your project. I hope that this helps.
