Poor performance with 4K HEVC - macOS

Playback of 4K H.265 (HEVC) footage from certain cameras (e.g., iPhone 15 Pro, DJI Mini 4 Pro) is unusable—severe stuttering and frame drops—in a new or existing sequence, even on powerful M2/M3 Mac systems. This occurs even with playback resolution set to 1/4 or 1/8. The same media plays smoothly in QuickTime and in previous versions of Premiere Pro (e.g., 24.x).

Many users are having issues with VFR. If the file is variable frame rate.
Use Handbrake to convert to constant frame rate before importing into Premiere Pro:
