Sound Issues in Premiere Pro

I am having reoccurring sound issues in Premiere.  Most times when I bring up another window (Chrome, Itunes, etc.) or import a file and then go back into Premiere, the sound becomes unsynchronized.

Another thing is when playing back in the timeline, the sound drops out all together.

Some context:

This is a work computer. I am using Mac Pro with an AJA io4K box plugged into a Yamaha OIV 96 sound board.

I have contacted AJA several times and they are still trying to help me out.

I didn't set this up at work.  All this was set up before I started. 

Any ideas?
If more information is needed, let me know, and I will try to help explain more.

I am a little unsure of the AJA box.

    I'm having this issue too.  I created a new project file, and imported everything, created a multicam timeline, and began editing.  When I closed Premier for the night, and came back to it the next day, there is now no audio playback in the sequence.  All the waveforms are there, but the volume meters don't register output and no audio comes through my output device.  If I go into a source clip and play it in the source monitor, audio plays fine.  I've checked all the output settings and nothing is obvious.  If I create a new project file and re-import all the assets, I can begin editing as normal, but as soon as I save and quit PP, the problem comes back again next time.  I'm at my wits' end!
