Zebra Lines on Clip in Sequence

Some clips have these grey and white diagonal lines on them and only the audio of the clip can be heard. What's the fix for this? The media isn't red so it's online and linked, in some capacity.

-Additionally, is there a way to see when a sequence-- you have multiple-- was created? Like a meta logging option that shows date?
