And after you go to Ann's location ... the Program monitor controls icon dialog ... you simply drag it anywhere you want it on the Program montor's control block.

The proxy button is no longer visible in the program monitor. The plus sign does not show the proxy button.

 I see it in 23.4.0, just like Ann's screenshot.

 

I would try resetting preferences.

 

Edit: same for Beta 23.5.0.33. What version are you running?
