Can't change the topic title of linked Word Document

I am using RH9 and Word 2010.  I have WebHelp set as the primary layout.  When I link some Word documents the topic title says "Chapter" and is grayed out so that I cannot change it.  I have gone to Project Settings and selected Heading 1 for pagination.  I've also tried various topic name patterns in Other Settings.

I can change the name in the TOC, but when it is generated it reverts back to "Chapter". 

One file I linked is retaining the topic name the same as the file name and I can't figure out what is different about this particular file.
