chm output is displaying displaying differently in RH2015

I just installed RH2015 yesterday. I generated my CHM file and it is displaying with a Glossary and Bread crumbs. I didn't have either of these before and don't want them. I'm attaching some screen shots.

First is my CHM from RH10, prior to upgrading to RH2015.

Next is what my CHM looks like now that I've upgraded to RH2015.

Next is a shot of my Window settings and output settings. I do not have glossary or bread crumbs selected. Can anyone help me fix this?!!

Hi there

First off, have you tried clicking Help > Updates?

Once the updates have been applied, your version from Help > About should show 12.0.2.384.

    Oh, and I believe you may be confusing Breadcrumbs with Browse Sequences.

    This is a Browse Sequence bar.
