Content categories and context-sensitivity

Has anyone successfully implemented context sensitivity with content categories? Context sensitivity worked fine for us in the past, but now that we've changed to content categories and thus the folder structure has changed, I cannot get it to work. Previously, our code was calling the index_csh.htm in the root folder. However, now that file does not exist in the root folder and instead there a [ContentCategoryName]_csh.htm file in each of the content category folders.

Advice?
