How to get Index to appear in Table of Contents

Using RH 2022.3.93

When I generate a PDF from RH, there's no Index within the Table of Contents.
I would like this to show up in the TOC:

Chapter 1. Introduction
Chapter 2. Customization
Chapter 3. Using
Index

Index does show up in the manual, but just not within the Table of Contents.

How do I get it to show up?

    By including it in Back Matter. See RH2022 The Table of Contents (grainge.org)
