Images missing in PDFs generated from Create PDF for Review

After generating a PDF for review, there are a couple of images that are missing:

• A GIF that is a GPS 'point' and that is used as part of a list style.
• A note PNG that is used in a 2-cell Note table. The icon is in the first cell, the note in the second.

Anyone have this experience and find a resolution to the problem?
