PDF Gaps in TOC

Hi,

When I generated a PDF, there are gaps in the TOC. In this case, between pages 234 and 235. The topics are very simple pages with no special HTML that I can see.

On another project, there is a gap of two blank pages in the middle of the TOC. 

Any idea what's causing this? Thanks.
